United HealthCare, Anthem Blue Cross Blue Shield, Ohio Medicare, and Ohio Mediaid.
What is the cancellation policy?
Please provide at least one business day ahead of scheduled appointment for cancellation. If you do not show up to your scheduled appointment, you will be invoiced for a $50.00 no show fee.
Why am I being asked for my insurance ahead of my appointment?
Some services provided required prior authorization and may not be covered by your insurance; we ask for your information prior to your appointment to asure your insurance covers the services being rendered.
Do I need a medical release to get medical records?
Yes, we require a signed medical release from your doctor or hospital to release any records.
